How can a support planner help me?

A support planner is able to:

  • Provide information about CDCS and provider options

  • Help you develop a community support plan

  • Help you monitor your support plan

  • Help you purchase and receive services and supports

  • Help you recruit, screen, hire, train, schedule, monitor and pay support workers

  • Help you solve problems that may occur

  • Provide and arrange for staff training specific to your needs

*Individuals receiving support planning services cannot contract for other services from Ramaker Consulting.

  • Positive Support Services (Positive behavioral supports)

At this time we are private pay only, which can be done through your FMS (Fiscal Management Services) or Approval Option (pass-though) until our 245D licenses is approved.

CDCS support planner: An individual selected by the person and reimbursed through the person’s CDCS budget to help develop and implement the person’s person-centered CDCS Community Support Plan (CSP). The support planner may work independently or be employed by an agency.


About chad

Chad has over 20 years of experience with disabilities

He is a licensed LGSW with a Masters' of Social Work from the University of Minnesota and a BA in Social Work and Psychology from Bethel University.  Most recently he was a County Supervisor for Minnesota Prairie County Alliance for 8 years supervising the Developmental Disability Waiver, SILS, Grant Programs, and Adult Licensing.  He spent 11 years providing 245D residential services and 5 years managing HUD housing and harm reduction housing.  

Chad is certified in Positive Behavioral Supports.  Trained in person-center planing through the University of Minnesota, Institute on Community Integration.  He also has training from the Humphrey Institute, Future Services, Equity Works Curriculum. 

He was previously a Certified Crisis Prevention Institute trainer

In the past, he has been a volunteer coach.  He currently services as the Ward 6 Park Board representative.
